How to create a form

With Radio Base you can create custom forms to share with your community and receive information and feedback from your users. You are able to build forms for event registrations, receiving feedback, and much more!


Step-by-Step Instructions:

Step 1: In your Dashboard, select the Content module in the dropdown menu. Scroll down and select Forms in the side panel menu and select Add Form.

Step 2: Add a title and description. Select Next.

Step 4: Now that the form is created, it will need to be configured. The three tabs to customize your form are Fields, Settings, and Entries.

  • Fields allows you to customize what information will be included in the form.

  • Settings includes the options for how the form will appear as well as setting up confirmations and notifications.

  • Entries provides a list of users that have completed the form.

Let's take a look at Fields. On right side of screen, select which Standard Fields and Advanced Fields you would like on your form. When selecting the fields, a preview will appear on the left side. When you are done with the field selection, select Save.

You have now created a form!


Automatic Confirmation

If you would like to have an automatic confirmation sent directly to the person who filled out the form, here's how! Keep in mind, users will need to provide an email address in order to receive the confirmation.

Step 1: Select the Settings option on the edit form page. Select Confirmations. Here is the default confirmation. The slider activates or deactivates the confirmation. The name, type, and content of the confirmation are also listed. Lastly, the edit icon may be found under the action column.

Step 2: Select Edit to customize the confirmation.

Step 3: Once you have added the type of confirmation and message, scroll down and select Update Form Confirmation.

Now users will receive a confirmation once they have completed the form!
